I did 2 forms using Google Forms. One form is actually a lesson in itself. It focuses on what career the student wants to do. It walks them through questions that will be saved on forms for the teacher to utilize. The teacher can use the information to plan future lessons, field trips, or classroom visits. The teacher can also use it for IEP purposes related to transition. The title of the form is "What are My Career Choices?" When I did the self checking quiz I tried to utilize a new form that we may implement for our Juniors.  It is a Junior Checklist.  It asks the students questions related to postsecondary education.  It includes questions about assessments, options/interests, clubs/organizations, college entrance exams, meeting with counselor to review credits, understanding their disability, etc.  These questions will then be self graded and the teacher will be able to see all of their students scores and look at the ones that they still need to do, which will help the student, teacher, and counselor.  The document is titled "Junior Checklist".  My plans are to make both shared to all sped teachers so that they can see what their students interests and needsSchoolCenter Picture.
